Medical Authorization and Release

The children named during registration have my permission and consent to participate in the Aquadillo practices and other activities related to training and competition in North West Swim Circuit (NWSC) swim meets. In the event of illness or injury to said children and in the event a parent or guardian is not present, and after an attempt has been made to inform them of such injury or illness, either the team coach(es) or official chaperone(s) are hereby authorized to obtain treatment by a medical doctor for said children as a parent or guardian could authorize if he/she were present. The undersigned hereby release and agrees to hold harmless, except in the case of negligence, Georgetown Summer Swim Association, its Board of Directors, coaches and official chaperone(s) from any and all claims, liability, cost and expense arising out of or resulting from participation in any such training activity, competition or from the procurement of medical treatment for the above listed swimmers. This authorization and release shall remain in effect until such time as it is terminated by written notice to the Board of Directors and Georgetown Summer Swim Association.

Photo Release

I, being the legal guardian of parent of the minor(s) listed during registration, hereby authorize and release from any and all liability GSSA, its coaches, agents and Board to use photographs of my family taken at GSSA functions for use on the GSSA web page or for use in publications promoting GSSA. No names will be used to identify individuals.

NWSC Code of Conduct

The Northwest Swim Circuit was formed with the objective of helping young swimmers to develop: physical fitness, self discipline, self confidence, proficiency in competitive swimming strokes, a determination to do one's best and good sportsmanship. The expectation of the Executive Board is participants will exhibit the highest standard of sportsmanship at all times while participating in all NWSC and member club affiliated events. Every participant, including swimmers, coaches, officials, board members, parents and spectators, has a responsibility to behave in respectable manner and to comply with and support the Code of Conduct.

While attending and/or participating in any NWSC or Member club affiliated practice or event, no individual shall:

If this individual is acting in the capacity of a coach or meet official at the time of the offense then the additional sanctions will be as follows:

*Any violation will be reported in writing to the vice president of the NWSC Executive Board either by electronic or traditional mail prior to the next NWSC sanctioned event or within 30 days of infraction if occurrence is at the last sanctioned event for the swim season.

Family Responsibilities

Volunteer Obligation

The Georgetown Summer Swim Association (GSSA) operates through the volunteer efforts of parents. This allows us to keep membership fees as reasonable as possible. Each family supports this volunteer network by actively participating in the competitive meets. If your child is swimming at a meet, you must also sign-up for a volunteer position at that meet. Most of the volunteer activities are easy and require not previous experience! Parental participation is required in order for your child to participate in each swim meet. Without adequate parental participation in our meets, part-time paid help would be required. Georgetown is admired throughout the circuit for our volunteer system and the efficiency of our meets. We want to work hard to keep it that way! "Opt- out option": You may opt-out of working the meets for a $250 fee. This fee can be paid during registration. If you don't opt-out and fail to fulfill your volunteer requirement, you will be charged the $250 opt-out-option fee.

Fundraiser Obligation

Fundraising is an essential element of most, if not all, summer swim programs, and helps defray the cost of coaching and pool rental fees. The Aquadillos Board of Directors Requests that your family participate in fundraising activities to the best of your abilities. The primary fundraising event this year will be a "Dillos Splash Bash" (date and time to be determined). There will be games, a raffle, a small silent auction, food, swimming, and fun for the whole family. We will also have several team-building events at local restaurants to foster camaraderie and team spirit. These will be "passive" fundraisers in that your contribution is purchasing dinner and the local restaurant give Aquadillos a portion of the total receipts. Dates and locations will be announced later.

Handbook

I accept the responsibilities to familiarize myself with the contents of the website and Parents Handbook. The handbooks will be distributed on the website and, upon request, at registration and practice.
